Our client, a dedicated social welfare organization, is seeking a compassionate and organized Social Care Case Manager to join their vital services team in Manama, Capital, BH . This role is fundamental to providing comprehensive support and advocacy for individuals and families facing various social challenges. The ideal candidate will have a strong commitment to social justice, excellent interpersonal skills, and a proven ability to manage complex cases with empathy and professionalism.

Key Responsibilities:
  • Conduct initial assessments of clients' needs, including social, emotional, financial, and physical well-being.
  • Develop individualized care plans in collaboration with clients, setting achievable goals and identifying necessary resources.
  • Coordinate and connect clients with appropriate internal and external services, including counseling, housing, medical care, employment assistance, and legal aid.
  • Monitor clients' progress regularly, adjusting care plans as needed to ensure effective outcomes.
  • Advocate on behalf of clients to access benefits, services, and rights they are entitled to.
  • Maintain accurate and confidential client records, documenting all interactions, assessments, and referrals.
  • Collaborate with other social workers, therapists, and community partners to provide holistic support.
  • Conduct home visits or community outreach as required.
  • Provide crisis intervention and emotional support to clients in distress.
  • Prepare comprehensive reports on case progress, outcomes, and challenges for supervisors and funding bodies.
  • Stay informed about changes in social welfare policies, available resources, and best practices in case management.
  • Participate in team meetings, case conferences, and professional development training.

Qualifications:
  • Bachelor's or Master's degree in Social Work, Psychology, Sociology, or a related human services field.
  • Minimum of 3-5 years of experience in case management, social work, or counseling, preferably within a community or non-profit setting.
  • Strong understanding of social welfare systems, community resources, and challenges faced by vulnerable populations.
  • Excellent active listening, empathy, and communication skills (verbal and written).
  • Ability to build rapport and trust with diverse client populations.
  • Strong organizational skills, with the ability to manage a caseload effectively and prioritize tasks.
  • Proficiency in case management software and Microsoft Office Suite.
  • Ability to handle sensitive information with confidentiality and discretion.
  • Resilience, patience, and a non-judgmental approach.
  • Bilingual proficiency (Arabic and English) is a significant advantage.
  • A valid driver's license may be required.
