- Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required; Advanced Practice Registered Nurse (APRN) or relevant clinical degree preferred.
- Minimum of 5 years of experience in a clinical setting, with at least 2 years focused on care coordination, case management, or telehealth.
- Demonstrated experience with chronic disease management programs.
- Proficiency in using electronic health records (EHR) and remote patient monitoring (RPM) platforms.
- Excellent verbal and written communication skills, with the ability to build rapport and trust with patients remotely.
- Strong organizational and time management skills, with the ability to manage multiple priorities effectively.
